Our Model

We identify challenges early and prevent others from happening, so that young children have a strong foundation for a lifetime of healthy development, learning, and growth.

A New Expert on the Pediatric Primary Care Team

We achieve improved outcomes for young children and families by bringing together the expertise of a child development expert, the HealthySteps Specialist, and the pediatric primary care provider.

Together, these experts bring focus to the important interwoven skills needed for families to foster healthy child development and life-long well-being: social-emotional development; language and literacy skills; cognition skills; and perceptual, motor, and physical development.

Power of Relationships

HealthySteps Specialists build strong relationships with families and providers so that communities can ensure all babies have a strong start during this critically important time in their life.

Tiers and Core Components

The evidence-based HealthySteps model is organized into three Tiers of Service and eight Core Components to make sure all families in the practice with children ages 0-3 receive support aligned with their needs.
